About Us

Second Chance Home Care, LLC was established to provide individuals and their loved ones with personalized in-home services where our clients can remain in the comfort of their home and still receive great quality care. Second Chance Home Care provides Home Care services in Hartford and Tolland counties in Connecticut. We are dedicated to maintaining the dignity, comfort, independence, well-being, and happiness of each client.  We do this by providing the highest quality professional caregivers at an affordable cost. We strive not only to assist our clients with home care services but also educate in self-awareness and how to use self-care techniques to continue to maintain a healthy lifestyle between home visits from our caregivers. Our goal is to make home care an affordable alternative for clients by taking the time to listen to our client’s short-term and long-term needs regarding their in-home care options.

Personal Care

Our Personal Care Attendants assist you with hands-on non-medical needs. While some of the tasks they perform are similar to those of our companions they provide additional services such as assistance with walking and medication reminders.


Our Homemakers help manage your household while performing general housekeeping duties. Our Housekeepers provide professional in-home cleaning and household chores only; meal preparation is not included. They are available to assist you 24 hours, 7 days a week.

Live In PCA

Our caregivers provide 24 hour personal care services. They aid in maintaining a comfortable home environment while building long lasting relationships with each client.

Geriatric Care Management

Geriatric care management services can help with developing, monitoring and modifying a care plan to assist with day to day activities, arranging and monitoring in-home help or services, and many other aspects of care management

Autism Respite Services

Second Chance Homecare Autism Respite Services provides a periodic, short-term break for caregivers of individuals with autism or autism disorders and their families. That environment can be the family home or in the community at specific times or for specific purposes. Our respite services are customizable and flexible, focused on the individual needs of the autistic person and the family. These services can include personal care, assistance with the activities of daily living, and socialization skills. We serve autistic individuals of all ages and their families. We are CT licensed #HCA 000607 to deliver the best care.

Group Day Services

We provide support to participants that want to prepare for work and or community participation. We strive to create a safe environment which facilitates newfound friendships. In addition, we provide engaging activities which will lead to greater socializing amongst their peers.

Group Supported Employment (GSE)

SCHC provides support to our participants in a structured work environment as a group. This program also includes assisting the participants with assessment, career planning, locating a job or developing a skill.

Individualized Day Support (IDS)

Tailoring to the individual’s own capabilities, we provide a one to one service centered on their goals and interests. This can include vocational training, recreational pursuits and opportunities to improve their social skills.

Individualized Supported Employment (ISE)

SCHC provides ongoing support related to the improvement and retention of skills to prepare an individual for work. The provider will have a 1 on 1 conversation, to truly draw out someone’s latent potential.

Individualized Home Supports (IHS)

SCHC assists with developing skills that can facilitate independent living. This includes improving domestic skills, and physical therapy regimens that strengthens mobility and flexibility.

Personal Support (PS)

SCHC provides services that are necessary to meet the participant’s day to day activity needs. This includes attending to the personal daily needs of the participant, such as feeding and hygiene. In addition, we have a registered nurse to assist with medication if needed.

Respite Services

SCHC ensures that the individuals receiving respite services are safe in a safe environment and they remain at optimal health. We understand that our participants and their families may feel overwhelmed, therefore, SCHC offers more hands-off care if it suits the individual’s needs.

Senior Support

SCHC provides senior support based out of the participant home for individuals who are 62 years old and older. Senior participants are eligible for all services SCHC provides.


We provide transportation to and from the participant’s residence to their chosen destination. We strive to ensure that everyone can participate in our activities.

The Team To Trust

Our caregivers are thoroughly screened via background and reference checks. Also, all of our caregivers are certified in CPR and first aid. At Second Chance Home Care, we directly employ all of the caregivers; we do not use independent contractors. We are state licensed, bonded, and insured.
