Our Home Care Services
Personal Care Assistants
Our Personal Care Assistants help you with hands-on non-medical needs. While some of the tasks they perform are similar to those of our companions, they also provide:
- Assistance with bathing, dressing and/or feeding
- Assistance with walking and transfers
- Medication reminders
- Meal preparation, planning and shopping
- Light housekeeping
Homemaker/Housekeepers
Our Homemakers help manage your household while performing general housekeeping duties. Our Homemakers help with:
- Meal preparation (only included with Homemaker)
- Changing linens
- Bathroom cleaning
- Vacuuming
- Dusting
- Washing dishes
- Cleaning
- Ironing
We Now Offer Autism Respite Services
Companion Sevices
If you need light assistance with activities of daily living and would enjoy a little company around the house, our Companions can help. Some of the tasks Companions perform include:
- Medication reminders
- Supervision of dressing and bathing
- Assistance with phone calls and correspondence
- Stimulating conversation
- Participation in leisure activities
- Routine check-ins to ensure that all is well
- Assistance with client mobility
- Meal preparation
- Laundry
- Bed making
- Involvement in leisure activities
Live-in PCA
Our Live-In Companions live in your home for 24 hours per day, 7 days per week. They aid in maintaining a comfortable home environment while building long-lasting relationships with each client. Among the duties performed by Live-In Companions are:
- Meal preparation
- Household chores
- Reading to clients
- Grocery shopping
- Other activities similar to those provided by our Companions, Homemakers and Personal Care Assistants.
Geriatric Care Management Services
- Develop, monitor and modify a care plan to assist with day to day activities.
- Arrange and monitor in-home help or services.
- Act as a liaison to families at a distance.
- Provide a link to community resources available to the client.
- Review financial, legal, or medical issues and refer to appropriate resources.
- Assist with town, state and federal applications (i.e. Medicaid applications).
- Provide counseling and crisis intervention to address emergency situations.
- Assist with alternative living arrangements when home is no longer an option.
- Provide support and counseling to families.
- Coordinate modification of home living.
- Telephone Consultations
Group Day Services
We provide support to participants that want to prepare for work and or community participation. We strive to create a safe environment which facilitates newfound friendships. In addition, we provide engaging activities which will lead to greater socializing amongst their peers.
Group Supported Employment (GSE)
SCHC provides support to our participants in a structured work environment as a group. This program also includes assisting the participants with assessment, career planning, locating a job or developing a skill.
Individualized Day Support (IDS)
Tailoring to the individual’s own capabilities, we provide a one to one service centered on their goals and interests. This can include vocational training, recreational pursuits and opportunities to improve their social skills.
Individualized Supported Employment (ISE)
SCHC provides ongoing support related to the improvement and retention of skills to prepare an individual for work. The provider will have a 1 on 1 conversation, to truly draw out someone’s latent potential.
Individualized Home Supports (IHS)
SCHC assists with developing skills that can facilitate independent living. This includes improving domestic skills, and physical therapy regimens that strengthens mobility and flexibility.
Personal Support (PS)
SCHC provides services that are necessary to meet the participant’s day to day activity needs. This includes attending to the personal daily needs of the participant, such as feeding and hygiene. In addition, we have a registered nurse to assist with medication if needed.
Respite Services
SCHC ensures that the individuals receiving respite services are safe in a safe environment and they remain at optimal health. We understand that our participants and their families may feel overwhelmed, therefore, SCHC offers more hands-off care if it suits the individual’s needs.
Senior Support
SCHC provides senior support based out of the participant home for individuals who are 62 years old and older. Senior participants are eligible for all services SCHC provides.
Transportation
We provide transportation to and from the participant’s residence to their chosen destination. We strive to ensure that everyone can participate in our activities.
We do this by engaging our clients through various activities such as gardening, birthday parties, and other social events.
